Yes. We are a New York State Licensed Home Care Agency operating throughout Long Island and the 5 boroughs.
Yes. All our personnel are bonded and insured.
Yes. Once you decide on services, one of our registered nurses will come to your home, coordinate with the patient’s physician or Medicaid Plan, and develop a customized Plan of Care.
Our Personal Care and Home Health Aides are New York State-certified aides who have undergone a rigorous state-sanctioned training program. Periodically, the aides receive ongoing in-service training and care assessment.
Yes. Reliance Home Senior Services works with MLTCs, CDPAP, Traumatic Brain Injury (TBI), and the NHTD Waiver Program. We also work with the Veteran’s Administration.
The Medicare Program is very restrictive regarding home care. With a physician’s consent, the patient must be discharged from a hospital or rehab facility and must also be in need of a skilled service – nursing, PT, OT or speech therapy. Once approved, aide services will then only be provided for a few hours per day for a limited duration.
After hours, our on-call coordinator is available 24 hours/day, 7 days/week.
We are very different. Reliance Home Senior Services is a NYS licensed agency providing personal care for its patients. By contrast, a Companion Agency may NOT provide personal care or activities of daily living. The aide may NOT touch the patient – there is no personal care permitted per NYS law. The only purpose of the Companion Agency is to keep the patient company.
If at any time you wish to change aides on assignment with you, simply call your coordinator at Reliance Home Senior Services and we will immediately honor your request. Reliance Home Senior Services has a large availability of qualified aides…changing aides is not an issue.
