Full-service wellness program – Every new patient will be evaluated by our RN and SW in order to get a full picture of patient needs. If needed, the patient will be receiving a phone call and or visit by one of our nursing staff. We feel with proper oversight we can help prevent and minimize hospitalization give needed support to our caretakers.
Education For Caretakers
Mentorship Program
The Home Health Aide Peer Mentorship Program is a job shadowing program that helps newly hired Home Health Aides (HHAs) to gain comprehensive knowledge and practice under the guidance and coaching of an experienced home health
aide.
Eligibility for this program is determined at orientation and onboarding. Ideal program participants are:
- Recent graduates of an HHA certificate program.
- Individuals transitioning from nonhealthcare industries.
- Incumbent HHAs who need, or who have expressed interest in, additional training for specific areas.
Reliance also has bi-monthly education and in-service programs where the home health aides are given information and instruction on current diseases and outcomes.
Medicaid Applications
For families who require assistance obtaining community Medicaid for their aging loved one, Reliance offers expert application services that are proven to lead to acceptance the first time around. Reliance hopes to become community’s go-to application center.
Reliance is also proud to assist families in setting up Chronic Care Medicaid, which is designed for those entering a nursing home.
Reliance can also help families set up a pool trust for their loved one, should such measures be necessary. By making both services accessible to families of any means, Reliance strives to ensure that every elderly member of the community is covered.
Enrollment Services
Once our clients are approved for Medicaid, Reliance serves as the liaison between the patient and Maximus, scheduling the initial evaluation appointment with any special needs in mind. From there, Reliance will also coordinate any MLTC visits that need to be made to the patient’s home to determine eligibility. These enrollment services are provided completely free of charge to all patients, and our clients are typically enrolled within 2-3 months of beginning the Medicaid application process.
Pending Care
For patients with a pending Medicaid application, Reliance will establish Medicare coverage for bridge services through our network of trusted CHHAs. Patients can expect to receive 4-5 care hours per day during this phase of the Medicaid application process.
Geriatric Management & Social Work Services
Reliance boasts a well-rounded roster of social workers that provide a myriad of in-home services to our patients. From pre-pouring medications and setting up doctor’s appointments to managing mail and shopping needs, the social services we provide ensure that patients and families are as free from stress as possible.
